AUHSD Physical Form is required, even for Kaiser patients. You must have your doctor complete the AUHSD Physical Form.

• The 2 AUHSD sports forms must be completed before you can attend practice (Practice begins June 1 and you must have forms in-hand to attend your first practice)

• Physical form must be completed before you can participate in physical contact football practice. Please plan to complete your physical forms by June 15th, preferrably sooner.
